We understand that it is not always possible or easy for you to come to us, so we can come to you. Our experienced staff can complete a comprehensive assessment of your needs and tailor a program for you to achieve your goals in the comfort and convenience of your own home or care home. Home visits allow us to see how you manage your daily tasks at home and we can give support and advice on how to make these more efficient.
Treatment Plan
During the home visit we will complete a full assessment of your physical movement, mobility and balance. We also will assess falls risks and elements of your environment for safety. We will provide a plan of care which may include
- Education and advice regarding safety and falls risk
- Home exercise programme
- Mobility aids assessment and advice
- Transfer advice and practice
- Hands on treatment
- Pain relief
